Payment of Fee: Candidate should pay the fee in the prescribed challan form. After filling up the required information on the Challan Form, they should make payment of the fee applicable to them in any branch of Canara Bank for credit of current account No. 0506201003871 maintained with Canara Bank, Main branch, Bellary (DP Code 506) and should keep the ‘Candidate’s Copy’ of the Challan with receipt of fees duly acknowledged thereon, with them for producing the same at the time of written test along with the latest passport size photograph pasted on the Challan and signed across by the candidate.
Selection Procedure : The selection will be made on the basis of performance in written test and interview.

How to apply
All eligible candidates should apply on-line from 12.01.2012 to 10.02.2012. After applying on-line, the candidates should take a print out of the system generated on-line application form and retain it for future reference.
a) A candidate must possess a Bachelor’s Degree or its equivalent from a recognised University. (b) A candidate who has appeared at an examination, the passing of which would render him eligible to appear at the examination but has not been informed of the result, may apply for admission to the examination. A candidate who intends to appear in such qualifying examination, may also apply provided the qualifying examination is completed before the commencement of this examination. Such candidates will be admitted to the examination, if otherwise eligible, but the admission would be deemed to be provisional and subject to cancellation if they do not produce proof of having passed the examination in any case not later than two months after the commencement of (Main) written examination, failing which their candidature shall automatically stand cancelled. (c) Candidates who have passed the final professional M.B.B.S. or any other Medical Examination equivalent thereto but have not completed their internship by the time of submission of their application for the HAS etc. examination will be provisionally admitted to the examination provided they submit along with their applications a copy of certificate from the University / Institutions that they had passed the requisite final professional medical examination. In such cases, the candidates will be required to produce, at the time of their interview, original degree or a certificate from the concerned competent authority of the University / Institutions that they had completed or requirement (including completion of internship) for the award of degree.
